Development and Management (LDM)

requires candidates to complete
the following forms:

Candidate Application
- Candidate Application
starts your process of
enrollment and must be
completed in order to be
enrolled in the program.
Students complete the
candidate application
process in order for a
student information is
listed in our curriculum
program. |

Eligibility Assessment
- Each candidate must
meet the eligibility
qualifications for
enrollment.
Because our University
is government training
university, students
must be employed by a
federal funded Head
Start program in order
to enroll. The
eligibility assessment
assure the University
that each candidate
meets those
requirements. |

Renewal Enrollment Form
- Previous credentials
and licenses are good
for 3 program years.
Credential and license
candidates must new
their credential for 2
more program years. |

Upgrade Enrollment Form
- This for is for those
students who has a
previous credential from
another program.
The Upgrade enrollment
form allows you to
transfer your credential
to a University
credential. |

Self-Appraisal Form
-
This form is designed to
help you summarize your
teaching and other
professional activities
and accomplishments
during the eligibility
assessment. You should
utilize this form to
present, on your own
behalf, all pertinent
information regarding
your effectiveness
and/or accomplishments
within the framework of
conducting training
events. Wherever
possible, you are
encouraged to submit any
supportive evidence
(documents, illustrative
materials, etc.) which
will augment your
evaluation. The form is
in two sections.
Sections one deals with
your teaching and
classroom related
activities, Section two
deals with your
professional activities. |

Registration Form -
This form is used to
register for training
classes. Use this
form along with course
listing at
www.courses.hsuniversity.org
to find the most updated
training classes. |
